INDIVIDUALIZED HOME SUPPORTS (IHS)

At A Place Called Home, we provide Individualized Home Supports tailored to the unique needs, goals, and preferences of each individual and their family. These services are designed to promote independence, build skills, and enhance quality of life through personalized support and training. As part of the Waiver Reimagine initiative, Individualized Home Supports combines several previously separate services into one flexible service, making it easier to access the right level of support and adjust services as needs change.

WITH FAMILY TRAINING

This service provides personalized support and training to help individuals develop skills, increase independence, and succeed at home and in the community. For adults, services focus on the individual's support and training needs. For children, services may support both the child and their family in building skills and achieving their goals. As part of the Waiver Reimagine initiative, Individualized Home Supports with Family Training streamlines previous in-home family support services into one flexible, comprehensive service that can be tailored to each family's unique needs.

RESPITE

Respite Care
Temporary care services that provide relief for primary caregivers while ensuring individuals continue to receive safe, compassionate, and high-quality support.

Primary Caregiver(s)
The individual(s) primarily responsible for the care, supervision, and overall well-being of the person receiving services.

In-Home Respite Care
Temporary care and support provided in the comfort and familiarity of the individual’s home, giving caregivers time to rest, attend to personal responsibilities, or recharge.

Away-from-Home Respite: Temporary relief administered outside the individual’s home or designated place of residence.

NIGHT SUPERVISION SERVICES

Night Supervision Services

Night Supervision Services
Overnight support and monitoring provided by trained staff to ensure the individual’s safety, well-being, and peace of mind while they remain in their own home.

Own Home
For night supervision services, an individual’s own home refers to a residence that is owned, rented, or leased by the individual or their family, and is not owned, operated, or leased by a service provider.

HOMEMAKER

Household Support Services

Household Support
Services designed to assist individuals with routine household tasks, helping them maintain a clean, safe, and comfortable living environment. Included   Services

Household support services may include light housekeeping, laundry, dusting, vacuuming, organizing living spaces, and other routine cleaning tasks that support daily living.

Eligibility
Household support services are available to eligible individuals living in their own homes or community settings.

Service Limitations
Household support services are not available to individuals who live in licensed residential settings, such as foster care or supervised living facilities, or those receiving adult foster care, child foster care, customized living, or 24-hour customized living services.
